Alternatively...if you'd rather use pen and paper contact Helen in the office on 020 7367 4122 or send an email to helen@napac.org.uk telling her what you'll be doing to fundraise for NAPAC and she'll create a personalised sponsorship form for you on NAPAC headed paper.
